Slide Competition Entry Form

I want to enter the slide competition at this year's Cali-Slide.

Rules to enter:

1. Age 18+ (by 11/07/25)
2. Have sliding skills
3. Attend Cali-Slide (11/08/25 is Slide Competition night)

No purchase is necessary to enter the competition. There is an admission price to attend the event.

Judges' scoring criteria tips: "Avoid falling and aim for a high level of difficulty."
